About the Management of Change (MOC) Form

Many process incidents trace back to small, uncontrolled changes: a set point raised, a trip bypassed, a transmitter replaced with a different range. Management of change makes sure every change is reviewed, documented, communicated and closed out. This form keeps an instrument MOC on one page.

How to fill in this template

  1. Describe the change, the reason and the tags affected; choose the type and whether it is temporary.
  2. Rate severity and likelihood from 1 to 5; the risk score and level are calculated.
  3. Mark each review item OK when done, Not OK if still required, or NA.
  4. List the implementation and close out actions with owners and due dates.
  5. The MOC status stays Open until every action is closed; also clear every Not OK review item before close out.

What the formulas do

  • Risk score = severity × likelihood.
  • Risk level: HIGH 15 and above, MEDIUM 8 to 14, LOW below 8 (adjust to your risk matrix).
  • MOC status = Open while any action is Open; close review items before closing the MOC.

Frequently Asked Questions

What is management of change (MOC)?

A formal process to review, approve, implement and document changes to plant equipment, software, procedures or set points, so risks are controlled.

Does replacing an instrument need an MOC?

Not for a like for like replacement. A different type, range, material or function is a change and needs an MOC.

Why track temporary changes?

Temporary bypasses and overrides are a common cause of incidents when they are forgotten. A temporary MOC has an end date and is reviewed until removed.
