Measured values
| Item | Required | Tolerance (±) | As found | Result | Unit |
|---|---|---|---|---|---|
| PT 1101 A trip point | 14.5 | 0.15 | 14.56 | PASS | bar g |
| PT 1101 B trip point | 14.5 | 0.15 | 14.41 | PASS | bar g |
| PT 1101 C trip point | 14.5 | 0.15 | 14.62 | PASS | bar g |
Response time
| Final element | Required max (s) | Measured (s) | Result | Remarks |
|---|---|---|---|---|
| XV 1101 fuel gas SDV | 3 | 2.1 | PASS | Full stroke, limit switch OK |
| XV 1102 fuel gas SDV | 3 | 2.4 | PASS |
Test steps
| No | Check item | Status | Remarks |
|---|---|---|---|
| 1 | Permit and bypass authorised; operations informed | PASS | |
| 2 | Initiator isolated and test equipment connected | PASS | |
| 3 | Trip simulated at the initiator (or process taken to trip) | PASS | Simulated with hand pump at transmitter |
| 4 | Logic solver trip and alarm received in DCS | PASS | |
| 5 | Final element moved to safe position | PASS | |
| 6 | Valve stroke observed full travel and limit switch feedback received | PASS | |
| 7 | Tight shut off checked (where required) | NA | Not required for this SIF |
| 8 | Reset and restore to normal; bypass removed | PASS | |
| 9 | Initiator returned to service and reading compared with other sensors | PASS |
About the SIF Proof Test Record
IEC 61511 requires every safety instrumented function to be proof tested at the interval used in its SIL calculation, and the results recorded. A good record shows the as found trip values and response times, not just a tick, so dangerous failures and drift can be tracked and the assumed failure rates checked against reality.
How to fill in this template
- Fill in the SIF details and the proof test interval from the safety requirements specification.
- Record each measured trip point with its required value and tolerance.
- Measure the response time of each final element with a stopwatch or the DCS sequence of events.
- Mark each test step PASS, FAIL or NA, and remark on anything abnormal.
- A dangerous failure found during the test must be reported, repaired and the SIF retested before being returned to service.
What the formulas do
- Trip point result = PASS if |as found − required| ≤ tolerance.
- Response time result = PASS if measured ≤ required maximum.
- Select the overall result FAIL if any item or step failed.
- Next test due = test date + proof test interval.

Frequently Asked Questions
What is a proof test?
A periodic test that reveals dangerous undetected failures of a safety instrumented function, so it can be repaired and its PFDavg kept within the SIL target.
Why record as found values?
As found results show the condition the SIF was really in. They are needed to calculate failure rates and to justify test intervals.
What if a proof test fails?
Treat it as a dangerous failure: apply compensating measures, repair, retest and report it in the SIS performance records.
